Indications
Flupenthixol:
o Schizophrenia and other psychotic disorders.
o Sometimes used for severe anxiety, depression (especially when psychotic features are present), and agitation.
Nortriptyline:
o Depression (major depressive disorder).
o Neuropathic pain (e.g., diabetic neuropathy, post-herpetic neuralgia).
o Chronic pain syndromes.
o Anxiety disorders and sometimes used off-label for insomnia and migraine prophylaxis.
Dosage and Administration
Flupenthixol 0.5mg:
o Initial Dose: 0.5–1 mg per day, often divided into 1–2 doses.
o Maximum Dose: Up to 6 mg per day depending on clinical response.
o Administration: Oral, with or without food. Best taken at the same time daily.
Nortriptyline 10mg:
o Initial Dose: Typically 10–25 mg per day, usually taken at bedtime to minimize drowsiness.
o Usual Maintenance Dose: 25–150 mg per day depending on the condition being treated.
o Maximum Dose: 150 mg per day for depression; lower doses are used for pain management.
o Administration: Taken orally, usually at night to reduce daytime drowsiness.
Precautions
Flupenthixol:
Pregnancy: Should be used only if necessary; may affect fetal development.
Liver/Kidney Disease: Caution needed, dosage adjustments may be required.
Elderly: May be more sensitive to side effects like sedation and hypotension.
Side Effects: Extrapyramidal symptoms (e.g., tremors, rigidity), drowsiness, dry mouth, weight gain, blurred vision.
Nortriptyline:
Pregnancy: Generally considered safe in pregnancy (Category C), but consult with a doctor.
Heart Disease: Use with caution in individuals with cardiovascular problems, as TCAs can cause heart arrhythmias.
Liver/Kidney Disease: Dose adjustments may be required.
Elderly: Increased risk of side effects, start with a lower dose.
Side Effects: Drowsiness, dry mouth, constipation, urinary retention, blurred vision, weight gain, and potential heart rhythm changes.
