1. Risperidone 3mg
Indications
- Schizophrenia (adults and children 13-17 years)
- Bipolar disorder (acute mania)
- Irritability associated with autism (children 5-16 years)
- Off-label: Treatment of anxiety or behavioral disorders in certain patients.
Dosage and Administration
Adults (Schizophrenia/Bipolar Mania): Start at 1-2 mg/day, increase gradually to 2-4 mg/day as tolerated. Typical range: 2-6 mg/day.
Children (5-17 years): Start with 0.5 mg/day, increase based on response.
Can be taken with or without food.
Adjustments: Lower doses for elderly or those with liver/renal impairment.
Precautions
Risk of metabolic changes (weight gain, diabetes, dyslipidemia).
Tardive dyskinesia (involuntary movements) and neuroleptic malignant syndrome (fever, muscle rigidity).
Caution in elderly (especially those with dementia-related psychosis).
Monitor for prolonged QT interval, orthostatic hypotension, and increased prolactin levels.
2. Trihexyphenidyl Hydrochloride 2mg
Indications
- Parkinson’s disease (as adjunct to levodopa therapy)
- Extrapyramidal symptoms (caused by antipsychotic medications, e.g., tardive dyskinesia, parkinsonism)
Dosage and Administration:
Adults: Start with 1-2 mg/day, increasing gradually to 6-10 mg/day depending on clinical response.
Can be taken orally with or without food.
Adjustments: Lower doses for elderly or patients with liver/renal issues.
Precautions:
Caution in elderly: Increased risk of anticholinergic side effects like confusion, dry mouth, constipation, urinary retention, and blurred vision.
Contraindicated in narrow-angle glaucoma.
Use with caution in patients with cognitive disorders, BPH, or urinary retention.
May cause drowsiness, dizziness, or memory impairment.
