TIS-20

TIS-20 is an oral isotretinoin capsule, a potent retinoid used primarily for the treatment of severe acne vulgaris (especially cystic or nodular acne) that has not responded to other therapies. Isotretinoin works by significantly reducing sebum (oil) production in the skin, preventing the clogging of pores, and decreasing inflammation, which is central to acne development.

Dosage and Administration

Route of Administration: Oral capsules.

Typical dose for adults: 20 mg once or twice daily, depending on the severity of acne and the individual’s response to the medication.

Initial dose for severe cases: Often started at 0.5 mg/kg/day, increasing up to a maximum of 1 mg/kg/day as necessary, split into 1 or 2 doses per day.

Duration of treatment: Treatment usually lasts for 16-20 weeks. A second course may be considered if acne persists or recurs after the first course.

Administration: Take with food to improve absorption. Swallow the capsules whole without crushing or chewing them.

Precautions

Teratogenic Effects: Isotretinoin is highly teratogenic (causes birth defects). Women of reproductive age must use strict contraception during treatment and for at least one month after discontinuing the medication. Pregnancy tests are mandatory before, during, and after treatment.

Monitoring: Regular blood tests are required to monitor liver function, lipid levels (cholesterol and triglycerides), and any changes in mental health (due to the potential for mood changes).

Liver Health: Caution should be used in patients with liver disease or those who have elevated liver enzymes.

Psychiatric Effects: Use with caution in individuals with a history of depression or psychiatric disorders. Some patients may experience mood swings, depression, or suicidal thoughts while on isotretinoin.

Skin Sensitivity: The medication increases sensitivity to sunlight, so sun exposure should be minimized, and sunscreen should be used to avoid sunburn.

Dryness: Isotretinoin often causes skin dryness, including dry lips, eyes, and skin. Regular use of moisturizers and lip balms can help manage these effects.

Contraindications

Pregnancy: Contraindicated in pregnancy due to the high risk of serious birth defects.

Hypersensitivity: Contraindicated in patients allergic to isotretinoin or any of its components.

Severe liver disease or high lipid levels (e.g., high cholesterol or triglycerides).

History of psychiatric disorders: Should be avoided in patients with depression, mood disorders, or suicidal thoughts.
