Amiodarone

Amiodarone is a medication primarily used to treat and prevent various types of serious heart rhythm problems (arrhythmias). It is classified as a class III antiarrhythmic agent and works by affecting the electrical activity of the heart to help maintain a regular rhythm.

  • Recommended Usage
Indication:

Amiodarone is indicated for:

  • Atrial fibrillation (AF) and atrial flutter: Used for rhythm control.
  • Ventricular arrhythmias: Including life-threatening conditions like ventricular tachycardia (VT) and ventricular fibrillation (VF).
  • Other supraventricular arrhythmias: Like paroxysmal supraventricular tachycardia (PSVT).
Dosage and Administration:

Initial loading dose: For urgent cases, amiodarone is typically administered intravenously (IV) or orally in high doses initially.

  • Oral: The initial oral dose may be 800-1200 mg/day divided into 2-3 doses for the first 1-3 weeks.
  • IV: A typical intravenous dose for the acute phase may start at 150 mg over 10 minutes, followed by a continuous infusion (typically 1 mg/min for the next 6 hours, then 0.5 mg/min).

Maintenance dose: After the loading dose, the maintenance oral dose is generally reduced to 100-200 mg/day.


Dosages should be individualized based on the patient's response and tolerance to the medication.

Precautions:
  • Liver dysfunction: Amiodarone is metabolized in the liver, so patients with liver disease should be monitored closely.
  • Pulmonary function: Can cause lung toxicity, including pulmonary fibrosis. Regular chest X-rays and pulmonary function tests are recommended.
  • Thyroid function: Amiodarone contains iodine and can affect thyroid function. Periodic monitoring of thyroid function is advised.
  • Bradycardia: The drug can cause a slow heart rate (bradycardia). Close monitoring is essential, especially in patients with preexisting bradycardia.
  • Electrolyte imbalance: Hypokalemia or hypomagnesemia should be corrected before starting amiodarone.
Contraindications:
  • Severe sinus bradycardia or heart block (unless a pacemaker is in place).
  • Hypersensitivity to amiodarone or any of its components.
  • Severe liver disease or active hepatic dysfunction.
  • Thyroid disorders (hyperthyroidism) due to the iodine content of amiodarone.
  • Pregnancy: Category D (should only be used if the potential benefit outweighs the risk to the fetus).
