Solifenacin

Solifenacin is a muscarinic receptor antagonist (anticholinergic) used primarily to treat overactive bladder (OAB) symptoms, such as urinary urgency, frequency, and incontinence. It works by relaxing the bladder muscles, reducing involuntary contractions and helping the bladder hold more urine.

  • Recommended Usage
Indications:
  • Management of type 2 diabetes mellitus in adults.
  • As an adjunct to diet and exercise to improve glycemic control.
  • May be used as monotherapy or in combination with other antidiabetic medications.
Dosage and Administration:
  • Overactive bladder (OAB): Solifenacin is prescribed to manage symptoms of OAB, which include:
    • Urinary urgency
    • Increased frequency of urination
    • Urge incontinence (leakage of urine due to a strong urge to urinate)
  • Neurogenic bladder: In some cases, it is used for bladder dysfunction caused by neurological conditions.
Dosage and Administration:
  • Initial dose: The usual starting dose is 5 mg once daily.
  • Maintenance dose: The dose may be increased to 10 mg once daily based on the patient’s response and tolerance.
  • Administration: Solifenacin should be taken once daily, with or without food, and should be swallowed whole (not crushed or chewed).
Precautions:
  • Bladder emptying: Solifenacin can decrease bladder emptying, which could lead to urinary retention, especially in patients with preexisting conditions like benign prostatic hyperplasia (BPH) or those at risk for urinary retention.
  • Glaucoma: Use with caution in patients with narrow-angle glaucoma, as it can increase intraocular pressure.
  • Cognitive impairment: Solifenacin may cause dry mouth, confusion, or blurred vision, so it should be used with caution in elderly patients or those with dementia.
  • Renal and hepatic impairment: Dose adjustment may be necessary in patients with severe kidney or liver disease.
Contraindications:
  • Severe urinary retention.
  • Uncontrolled narrow-angle glaucoma.
  • Severe gastrointestinal conditions such as gastric retention, ileus, or intestinal obstruction.
  • Hypersensitivity to solifenacin or any of its components.
