Indications:

Valaciclovir is prescribed for:

Herpes Simplex Virus Infections:

  • Genital Herpes: Used to treat initial and recurrent outbreaks of genital herpes.
  • Cold Sores (Herpes Labialis): Treats outbreaks of cold sores (oral herpes).

Herpes Zoster (Shingles):

  • Treatment for shingles, which is caused by the varicella-zoster virus, the same virus that causes chickenpox.

Chickenpox (Varicella):

  • Used to treat chickenpox in children and adults, reducing the severity and duration of the illness.

Prevention (Suppressive Therapy):

  • Genital Herpes: For chronic suppression to reduce the frequency of outbreaks and decrease transmission to others.
  • Prevention in Immunocompromised Individuals: Helps prevent HSV infections in people with weakened immune systems (e.g., HIV-positive individuals or organ transplant recipients).
Dosage:

For Adults:

  • Genital Herpes:
    • Initial outbreak: 1,000 mg twice daily for 7-10 days.
    • Recurrent outbreaks: 500 mg twice daily for 3-5 days.
    • Suppression: 500 mg once daily to reduce recurrence.
  • Shingles: 1,000 mg three times daily for 7 days.
  • Cold Sores (Herpes Labialis): 2,000 mg twice daily for 1 day (usually within 1 hour of the first symptoms).
  • Chickenpox: 20 mg per kg of body weight every 8 hours for 5 days.

For Children (2 years and older): Dosing for children varies based on the condition being treated and their body weight, so it's important to consult a pediatrician.

Renal Impairment: Dosage may be adjusted for individuals with kidney problems, as valaciclovir is cleared through the kidneys.

Precautions:
  • Kidney Function: Valaciclovir can cause kidney issues, especially in patients with pre-existing kidney disease. Regular monitoring of renal function is advised, and dose adjustments may be required.
  • Hydration: Ensure adequate fluid intake, as dehydration can increase the risk of kidney damage, particularly in older adults or those with compromised kidney function.
  • Pregnancy and Breastfeeding: While valaciclovir is classified as a Category B drug (generally considered safe during pregnancy), it should be used during pregnancy only if clearly needed. It passes into breast milk in small amounts, so consult your doctor if breastfeeding.
